{"id":8382,"date":"2025-02-22T10:00:41","date_gmt":"2025-02-22T08:00:41","guid":{"rendered":"https:\/\/eu.bd.com\/iv-news\/?p=8382"},"modified":"2025-06-04T13:44:46","modified_gmt":"2025-06-04T11:44:46","slug":"home-infusion-italys-h-vas-programme-supports-picc-care-at-home","status":"publish","type":"post","link":"https:\/\/eu.bd.com\/iv-news\/events-training\/home-infusion-italys-h-vas-programme-supports-picc-care-at-home\/","title":{"rendered":"Home infusion: Italy\u2019s H-VAS programme supports PICC care at home"},"content":{"rendered":"<p>Italy has one of the highest percentages of older adults in the European Union,<sup>1<\/sup> with approximately 24% of the current population over the age of 65.<sup>2 \u00a0<\/sup>At the same time, cancer incidence is increasing in the elderly population more than in the young population.<sup>3<\/sup><\/p>\n<p>In Italy, several government initiatives focus on improving support for family caregivers and personal helpers who provide care in the home setting, as well as programmes aimed at providing greater support to elderly and dependent persons being cared for at home.<sup>4<\/sup><\/p>\n<p>Strengthening home care is one of the goals of the care model outlined in Italy\u2019s <strong>National Plan for Recovery and Resilience<\/strong>,<sup>5<\/sup> which sets out a plan to increase the volume of services rendered in home care, targeted to patients over the age of 65 with one or more chronic and\/or dependent conditions.<sup>5<\/sup> One of the goals is to maximise the benefits of technologies including telemedicine, home automation and digitisation.<sup>5<\/sup><\/p>\n<p>This is echoed in the <strong>Europe\u2019s Beating Cancer Plan<\/strong>,<sup>6<\/sup> which highlights the important role of family and informal carers, and the toll taken on them in terms of the time required for care activities, and potential loss of income due to reduced work availability.<sup>6<\/sup><\/p>\n<h2>Study shows no significant difference in PICC complications for inpatients vs. home infusion patients<\/h2>\n<p>Dr Pinto shared the results of a study by Mielke et al who retrospectively analysed 522 PICC placements in 484 radiation oncology patients for complications and treatment- and patient-related factors during ambulatory and hospital inpatient use.<sup>7<\/sup> The study showed that complications can occur both in inpatients and outpatients, but no significant difference in the number of complications was detected between the two groups.<sup>7<\/sup><\/p>\n<p>Complications were similar between the cohort of outpatients (3.6 per 1000 catheter days) and the cohort of inpatients (4.8 per 1000 catheter days.)<sup>7<\/sup> The ambulatory patients received weekly PICC care, while the PICCs of inpatients were checked for complications at every drug delivery.<sup>7 <\/sup>&#8220;In cancer patients, the peripherally inserted central venous catheter is feasible and safe if routine care and management are planned,&#8221; said Dr Pinto.<\/p>\n<p>Dr Pinto also shared the results from a qualitative content analysis on the supportive care needs of adults living with a PICC at home.<sup>8<\/sup> This study showed that caregivers play an integral role, and that it is vital to include them in clinical education programmes, and support them so they can fulfil this role.<sup>8<\/sup><\/p>\n<p><strong>More on this topic: <\/strong><a href=\"https:\/\/eu.bd.com\/iv-news\/events-training\/picc-or-port-which-device-for-patients-with-cancer\/\">PICC line or port catheter: Which one for cancer patients?<\/a><\/p>\n<h2>Home infusion programme developed to support cancer patients at home<\/h2>\n<p>Dr Monica Pinto, Director, S.C Rehabilitation Medicine, Istituto di Ricovero e Cura a Carattere Scientifico (IRCCS), National Cancer Institute \u201cG. <strong> Sign up <\/strong>\n\t\t\t\t\t\t\t\t\t<\/a>\n\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"content\" style=\"display:none;\">\n\t\t\t\t\t\t\t<p><iframe title=\"Vascular access excellence: improving care &amp; reducing costs with outside hospital care\" width=\"640\" height=\"360\" data-src=\"https:\/\/www.youtube.com\/embed\/ZSAHVOsqaFc?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" class=\"lazyload\" data-load-mode=\"1\"><\/iframe><\/p>\n<p>Safeguarding quality of life is one of the primary goals of the H-VAS programme, along with reducing time spent travelling to the hospital, and requiring less time off work for patients and their families.<sup>9<\/sup> The patients\u2019 caregivers at home are coached on how to perform weekly peripherally-inserted central venous catheter (PICC) dressing changes, supported by hospital clinical staff who are available via teleassistance on smartphones.<sup>9<\/sup> Patients and carers also benefit from reduced exposure to potential healthcare-associated infections.<sup>9<\/sup><\/p>\n<p>This programme is a collaboration between BD and Dr Pinto\u2019s organisation.<\/p>\n<p><strong>More on this topic: <\/strong><a href=\"https:\/\/eu.bd.com\/iv-news\/news-innovation\/convenience-packs-streamlined-iv-catheter-care-in-the-hospital-and-at-home\/\">Convenience packs: streamlined IV catheter care in the hospital and at home<\/a><\/p>\n<h2>Four pillars of H-VAS<\/h2>\n<p>The H-VAS programme is based on four pillars, explained Dr Pinto:<\/p>\n<ol>\n<li><strong>Technology:<\/strong> to enable remote support from expert clinical staff during PICC maintenance in the home;<sup> 9<\/sup><\/li>\n<li><strong>Medical device<\/strong>: a dressing kit is provided with all the elements necessary for PICC care and maintenance;<\/li>\n<li><strong>Certified educational programme:<\/strong> caregivers are trained and certified;<\/li>\n<li><strong>Follow-up<\/strong>: a tool is used to monitor caregivers\u2019 clinical practice to ensure high standards of practice and patient safety.<\/li>\n<\/ol>\n<h2>Pros and cons of home infusion: From the patient\u2019s perspective<\/h2>\n<p>In her presentation, Dr Pinto outlined from her experience the opportunities and the barriers for patients in implementing a home infusion programme for patients with PICCs.<\/p>\n<h3>For patients, the opportunities include:<\/h3>\n<ul>\n<li>Saving travel time to the hospital and reducing travel expenses<\/li>\n<li>Reducing stress related to the hospital setting<\/li>\n<li>Reducing lost working days (for both patient and caregiver)<\/li>\n<\/ul>\n<h3>The barriers for patients include:<\/h3>\n<ul>\n<li>The emotional aspects<\/li>\n<li>Caregiver availability<\/li>\n<li>Fear of not being skilled enough<\/li>\n<li>The fear of losing the relationship with healthcare professionals (HCPs) in the hospital setting<\/li>\n<\/ul>\n<h2>Wider potential<\/h2>\n<p>While the H-VAS programme was created to support chemotherapy patients, the potential to help other patients is clear. &#8220;H-VAS is for cancer, but we can also use it in other diseases,&#8221; said Dr Pinto.<\/p>\n<h2>About WoCoVA<\/h2>\n<p>The 8th WoCoVA took place 17\u201319 April 2024 in Prague, Czech Republic.
