{"id":8283,"date":"2025-05-01T10:00:46","date_gmt":"2025-05-01T08:00:46","guid":{"rendered":"https:\/\/eu.bd.com\/iv-news\/?p=8283"},"modified":"2025-06-10T18:50:22","modified_gmt":"2025-06-10T16:50:22","slug":"launching-an-opat-service-at-swansea-bay-health-board","status":"publish","type":"post","link":"https:\/\/eu.bd.com\/iv-news\/events-training\/launching-an-opat-service-at-swansea-bay-health-board\/","title":{"rendered":"Launching an OPAT service at Swansea Bay Health Board"},"content":{"rendered":"<p>In Wales, several government initiatives are aimed at bringing healthcare services closer to people\u2019s homes, making it easier for people to access care, and improving the efficiency of hospital resources, including the most effective utilisation of hospital beds.<sup>1,2,3 <\/sup><\/p>\n<p>Frankie Thompson, Lead Nurse Vascular Access and OPAT Services, Swansea Bay University Health Board, presented the challenges and benefits of implementing an outpatient parenteral antimicrobial therapy (OPAT) service at the 8th <a href=\"https:\/\/wocova.com\/\">World Congress on Vascular Access (WoCoVA)<\/a>, held 17\u201319 April 2024 in Prague, Czech Republic.<\/p>\n<p>The OPAT service implemented at the Swansea Bay Health Board offers an example of how shifting services out of hospitals, and into communities, can benefit patients, care providers and hospitals.\u00a0 \u201cIt has emerged as a real critical tool for hospitals to tackle the challenges associated with a shortage of beds,\u201d said Ms Thompson. Ms Thompson\u2019s session covered the scope of the OPAT program, the initial challenges, the initiatives that supported the launch, and the outcomes.<\/p>\n<p><strong>More on this topic: <\/strong><a href=\"https:\/\/eu.bd.com\/iv-news\/news-innovation\/convenience-packs-streamlined-iv-catheter-care-in-the-hospital-and-at-home\">Convenience packs: streamlined IV catheter care in the hospital and at home<\/a><\/p>\n<p>Ms Thompson cited the United Kingdom\u2019s \u201cStart smart then focus: antimicrobial stewardship toolkit for inpatient care settings,\u201d which provides evidence-based guidance for secondary care clinicians and leaders, as a foundational starting point for the OPAT service. Devices used in the service include peripherally inserted central catheters (PICCs), midline catheters, extended dwell\/long cannulas and peripheral venous cannulas.<\/p>\n<h2>Benefits of the OPAT model<\/h2>\n<p>For patients, the benefits are clear: OPAT offers patient-centered care that can be tailored to fit patients\u2019 preferences and situations.<sup>5<\/sup><\/p>\n<p>In her talk, Ms Thompson explained that exact cost savings might be difficult to quantify, but she shared some examples of how benefits could be estimated for an institution:<\/p>\n<ul>\n<li>Number of days in hospital reduced or avoided<\/li>\n<li>Reduced pressure on the emergency department and assessment units<\/li>\n<li>Reduced risk of healthcare-associated infection<\/li>\n<\/ul>\n<h2>Initial challenges at Swansea Bay Health Board<\/h2>\n<p>In her presentation, Ms Thompson shared the challenges faced by the Swansea Bay Health Board in developing the OPAT program. These included a lack of understanding about OPAT or patient suitability; a lack of clear\/standard referral processes; and reluctance on the part of consultants to continue reviewing patients after discharge. Other challenges included microbiologists with little or no OPAT experience.<\/p>\n<p>Other considerations were medication supply challenges, the fact that community teams only work 8 a.m. to 8 p.m., the issues of discharging patients out of the local area and concerns on the part of some of the patients, who believed that the hospital was the safest place to be for someone with an infection, for example.<\/p>\n<p><strong>More on this topic: <\/strong><a href=\"https:\/\/eu.bd.com\/iv-news\/events-training\/danish-vascular-access-team-facilitates-shift-to-home-care\/\">Danish vascular access team facilitates shift to home care<\/a><\/p>\n<h2>Communication and standardisation key to rollout<\/h2>\n<p>Ms Thompson outlined the initiatives that helped to ensure a smooth launch for the OPAT service at Swansea Bay. This included vascular access device and OPAT training; the development of vascular access care bundles; the development of OPAT pathways to standardise practices; and the creation of an OPAT Working Group.<\/p>\n<p>The hospital pharmacy staff set up hubs in the community to supply medications to nurses; and information leaflets outlined the program for patients. Once the program was running, the team began to gather and record key OPAT metrics. Currently, they hold monthly\/quarterly multidisciplinary meetings for processes and guideline development, and weekly multidisciplinary team meetings to discuss caseloads.<\/p>\n<h2>About WoCoVA<\/h2>\n<p>The 8th WoCoVA took place 17\u201319 April 2024 in Prague, Czech Republic.
