{"id":8040,"date":"2024-09-16T10:00:53","date_gmt":"2024-09-16T08:00:53","guid":{"rendered":"https:\/\/eu.bd.com\/iv-news\/?p=8040"},"modified":"2024-08-29T13:40:11","modified_gmt":"2024-08-29T11:40:11","slug":"effective-iv-cannulation-training-is-essential-for-patient-safety","status":"publish","type":"post","link":"https:\/\/eu.bd.com\/iv-news\/scientific-publications\/effective-iv-cannulation-training-is-essential-for-patient-safety\/","title":{"rendered":"Effective IV cannulation training is essential for patient safety"},"content":{"rendered":"<p>Placing an intravenous (IV) cannula can be challenging for experienced doctors and intimidating for less experienced ones.<sup>1<\/sup> Cannulation errors occur for different reasons including insufficient training, user error, improper use of a cannula or inadequate maintenance.<sup>2<\/sup> Cannula use-related factors may lead to IV complications.<sup>3<\/sup><\/p>\n<p>Medical students must acquire a certain number of safety behaviours before graduating from medical school.<sup>4<\/sup> When future doctors learn to master technical procedures, this may help them ensure patient safety by minimising the risk of making errors.<sup>5<\/sup><\/p>\n<h3>Non-technical skills (NTSs) are also necessary to minimise human error.<sup>6<\/sup> NTSs include:<sup>6<\/sup><\/h3>\n<ul>\n<li>Cognitive abilities<\/li>\n<li>Attitude<\/li>\n<li>Social skills<\/li>\n<\/ul>\n<p>Once acquired, NTSs enable medical students to face challenging, constantly changing and unfamiliar settings.<sup>7<\/sup><\/p>\n<h3>A range of formats are available for training medical students on how to cannulate a patient:<\/h3>\n<ul>\n<li>Practice on other students<sup>8<\/sup><\/li>\n<li>Mannequins<sup>9<\/sup><\/li>\n<li>E-learning<sup>7<\/sup><\/li>\n<li>Virtual reality<sup>9<\/sup><\/li>\n<li>Augmented reality<sup>10<\/sup><\/li>\n<li>Etc.<\/li>\n<\/ul>\n<h2>Evaluating hybrid IV cannulation training<sup>4<\/sup><\/h2>\n<p>Carter et al. conducted a <a href=\"https:\/\/doi.org\/10.1186\/s41077-020-00136-y\">qualitative study<\/a> at the University of Plymouth\u2019s Peninsula Medical School on safety behaviours in medical students using video-reflexive ethnography (VRE), interviews and two simulation scenarios.<\/p>\n<h3>The study was organised in five steps:<\/h3>\n<ol>\n<li>First simulation<\/li>\n<li>E-learning<\/li>\n<li>Second simulation<\/li>\n<li>VRE questionnaire<\/li>\n<li>Interview<\/li>\n<\/ol>\n<p>Researchers recruited five female and five male final-year medical students to participate in the simulations, the second one occurring two weeks after the first. Two actors played patients and a student nurse was recruited to play the part of the nurse. The actors wore a specialised cannulation sleeve to simulate real-world cannulation.<\/p>\n<p>Before the simulations, actors were instructed to change their moods and get into awkward positions. Participants had to manage poor lighting and equipment layout. During each simulation, the medical students faced a certain number of human factor-related issues to handle. The second simulation was designed to be unpredictable compared with the first simulation.<\/p>\n<h2>Training with IV cannulation e-learning<\/h2>\n<p>After the first simulation, participants participated in e-learning training.<sup>4<\/sup> They learned about theory, took quizzes and watched videos for a deeply engaging experience.<sup>7<\/sup><\/p>\n<h3>The e-learning resource was primarily designed for:<sup>7<\/sup><\/h3>\n<ul>\n<li>Doctors<\/li>\n<li>Nurses<\/li>\n<li>Paramedics<\/li>\n<li>Other healthcare professionals<\/li>\n<\/ul>\n<h3>The e-learning resource was made up of a total of four modules:<sup>4<\/sup><\/h3>\n<ol>\n<li>\u201cPrepare\u201d<\/li>\n<li>\u201cProcedure\u201d<\/li>\n<li>\u201cPreserve\u201d<\/li>\n<li>\u201cCommunication skills and human factors\u201d<\/li>\n<\/ol>\n<p>The aim of these modules was to help reduce high IV complication and cannula failure rates.<sup>7<\/sup> Participants were asked to complete the fourth module.<sup>4<\/sup><\/p>\n<h3>The fourth module covered the following topics:<sup>4<\/sup><\/h3>\n<ul>\n<li>Why communicate?<\/li>\n<li>Communication skills<\/li>\n<li>Human factors and safety<\/li>\n<li>Best practices in communication<\/li>\n<\/ul>\n<p>The fourth module included the Swiss cheese model,<sup>4<\/sup> which is made up of four \u201cslices\u201d or barriers with potential \u201choles\u201d or failures in them.<sup>11<\/sup> For cannulation, these barriers against patient harm include good communication, infection prevention, documentation and adequate staffing.<sup>4<\/sup><\/p>\n<p>Examples of human errors that may lead to patient harm during or after cannulation include: inadequate communication with the patient, poor hygiene, inadequate written documentation and staff shortages.<sup>4<\/sup><\/p>\n<h2>Evaluating this hybrid cannulation training<sup>4<\/sup><\/h2>\n<p>After the second simulation, study participants were asked to watch videos of themselves during the simulations and fill in a questionnaire to compare how they performed at each simulation. This method, called VRE, helped participants analyse and reflect on their performance. After completing the VRE questionnaire and within 10 days of the second stimulation, they also participated in 15\u201325 minute interviews.
