{"id":7841,"date":"2025-01-03T10:00:22","date_gmt":"2025-01-03T08:00:22","guid":{"rendered":"https:\/\/eu.bd.com\/iv-news\/?p=7841"},"modified":"2025-06-10T18:44:47","modified_gmt":"2025-06-10T16:44:47","slug":"how-to-build-cvam-care-bundles","status":"publish","type":"post","link":"https:\/\/eu.bd.com\/iv-news\/events-training\/how-to-build-cvam-care-bundles\/","title":{"rendered":"How to build CVAM care bundles"},"content":{"rendered":"<p>It is estimated that up to 90 percent of people treated within a hospital in-patient setting receive an intravascular (IV) catheter.<sup>1<\/sup> While intravascular access is vital for the delivery of fluid and medications, complications from a failed IV catheter impact not only the patient, but the healthcare system as a whole. <sup>1,<\/sup><sup>2<\/sup> In particular, <a href=\"https:\/\/eu.bd.com\/iv-news\/ivpaedia\/#catheter-related-bloodstream-infection\">catheter related bloodstream infections (CRBSIs)<\/a> are a significant cause of morbidity and mortality within healthcare settings.<sup>3<\/sup><\/p>\n<p>So, how can you improve central vascular access management (CVAM) and CVAM care bundles within your own healthcare setting? Read on for tips from an Infectious Disease expert.<\/p>\n<h2>Vascular access teams<\/h2>\n<p>Whether vascular access is through a central line, a peripherally inserted central catheter (PICC) line, a port or a peripherally inserted catheter, a dedicated vascular access team (VAT) can improve outcomes arising from vascular access devices.<sup>4<\/sup> Generally, a well-functioning VAT is drawn from a range of disciplines such as infection control, nursing and medicine, and is entirely supported by hospital management.<sup>4,5<\/sup><\/p>\n<h2>CVAM care bundles: clarity and inspiration<\/h2>\n<p>To help a VAT work effectively and efficiently, evidence-based guidance is readily available. The good news is that you can devise a robust care bundle, which can be tailored to the needs of your own healthcare population, using existing recommendations. However, with the plethora of guidelines from a variety of reputable sources, how do you avoid being blinded by information overload?\u00a0 How do you identify which guidelines are best suited for your team and your patients?<\/p>\n<h2>Advice from the field<\/h2>\n<p>Professor Doctor Ilker Devrim, who works in the Department of Paediatric Infectious Diseases and is Chair of the Infection Control Committee, Dr. Behcet Uz Childrens Hospital \u0130zmir, Turkey, has some advice. Speaking at the <a href=\"https:\/\/lp.bd.com\/202306-MDS23-EU_EN-Macova_webinar-LP_LP-EN-01-MainLP.html\">BD MACOV@23 congress<\/a>, Prof Dr Devrim outlined the various ways their VAT and CVAM care bundles made significant improvement to the outcomes of hospitalised children and to their healthcare system in general.<\/p>\n<p>The purpose of this short blog is to give you a starting point from which to develop your own CVAM care bundles. To this end we can turn to Prof Dr Devrim\u2019s two over-arching rules:<\/p>\n<ol>\n<li>Always use the \u2018all or nothing\u2019 approach. So, if you are planning to emulate other care bundles, but even a single element contained within that bundle does not work for your healthcare facility, exclude that entire bundle from your options.<\/li>\n<li>In your care bundle, it is vital to specify guidance not just for the insertion of a vascular access device, but also for its maintenance.<\/li>\n<\/ol>\n<p>What follows is what worked for Prof Dr Devrim\u2019s team when devising their CVAM care bundles. In addition to the topics listed below, they focused on the correct use of checklists to ensure every patient received the same high standard of care. Prof Dr Devrim said it took up to 6 months to see the impact of the care bundle once implemented.<\/p>\n<h3>The education and training sessions at Dr. Behcet Uz Childrens Hospital covered:<\/h3>\n<ul>\n<li>Selection of catheter insertion sites with an eye to reducing the risk of infection \u2013 PICCs and midline catheters are good options<\/li>\n<li>Selection of the most appropriate catheter type, with the minimum possible number of lumens<sup>6<\/sup><\/li>\n<li>Daily review of the clinical need for vascular access<\/li>\n<li>Hand hygiene and aseptic technique<\/li>\n<li>Barrier precautions<\/li>\n<li>Skin preparation with antiseptic selection and single-use applicators<sup>7<\/sup><\/li>\n<li>Site dressings with transparent, chlorhexidine-impregnated products<\/li>\n<li>Catheter removal and replacement<\/li>\n<li>Replacement of IV administration sets<\/li>\n<li>Selection of needle free connectors, taking into consideration factors such as ease of use and price<\/li>\n<li>Flushing with single use prefilled9% saline syringes, using the pulsatile technique to help reduce the risk of internal biofilm formation<sup>8<\/sup><\/li>\n<\/ul>\n<p>The discussion panel followed Prof Dr Devrim\u2019s presentation with some additional advice which is summarised below.<\/p>\n<p>Patient safety is an ongoing goal. This means that rather than being a compliance tick box exercise, the implementation of your CVAM care bundles should be part of a continuous quality assurance cycle. This entails:<\/p>\n<ul>\n<li>Sharing information, including any problems that may arise<\/li>\n<li>Maintaining a healthy surveillance system to support your VAT practices<sup>5<\/sup><\/li>\n<li>Track specific outcome measures, such as the incidence of phlebitis<sup>5<\/sup><\/li>\n<\/ul>\n<p>Hopefully you are now inspired to develop your own VAT and care bundles.
