The benefits of ultrasound in peripheral IV catheter insertion<\/strong><\/h4>\n<p>Traditionally, peripheral IV catheter insertion follows a visual inspection and palpation of the extremity to locate the vein.<sup>4<\/sup> However, thanks to ultrasound guidance, healthcare professionals can identify what Dr van Loon calls \u201cinvisible veins,\u201d or veins that cannot be seen from above the skin with the naked eye.<sup>3<\/sup><\/p>\n<p>With this innovation, ultrasound-guided cannulation can result in higher rates of insertion success (81%) when compared to traditional methods (70%), according to one study in which Dr van Loon participated.<sup>4<\/sup><\/p>\n<p>\u201cMore interestingly,\u201d said Dr van Loon. \u201cWe found that ultrasound is especially beneficial in patients who are difficult to cannulate.\u201d<sup>3 <\/sup>Dr van Loon and his colleagues also found that ultrasound-guided peripheral IV catheter insertion:<sup>4<\/sup><\/p>\n<ul>\n<li>Reduced the number of punctures<\/li>\n<li>Reduced time needed to achieve intravenous access<\/li>\n<li>Increased patient satisfaction<\/li>\n<li>Did not affect the risk of complications<\/li>\n<\/ul>\n<p><strong>Learn more about the use of ultrasound-guided cannulation within vascular access teams: <\/strong><a href=\"https:\/\/eu.bd.com\/iv-news\/scientific-publications\/emergency-department-vascular-access-teams-how-impactful-are-they1\/\"><strong>Emergency department vascular access teams\u2013How impactful are they?<\/strong><\/a><\/p>\n<h4><strong>2. The learning curve: Training nurses on ultrasound-guided cannulation<\/strong><\/h4>\n<p>\u201cWe all know that simply placing an ultrasound probe on a patient\u2019s arm doesn\u2019t automatically result in the correct insertion of a vascular access device,\u201d said Dr van Loon. \u201cSo, what do healthcare professionals need to become competent in ultrasound-guided peripheral IV cannulation?\u201d<sup>3<\/sup><\/p>\n<p>To raise competency amongst nurses, Dr van Loon conducted a systematic literature review to devise an evidence-based training plan, which included:<sup>5<\/sup><\/p>\n<ul>\n<li><strong>1 hour of didactic training:<\/strong> Lectures focusing on ultrasound physics, vascular anatomy and the use of the ultrasound-guided cannulation technique<\/li>\n<li><strong>2 hours of hands-on training:<\/strong> Identification of veins on a live model without cannulating, followed by a cannulation practice session on a non-human tissue model<\/li>\n<li><strong>10 supervised cannulations:<\/strong> Life-case scenarios, supervised ultrasound-guided peripheral IV cannulations of veins on the upper extremity, including patients with known difficult IV access<\/li>\n<\/ul>\n<p>Dr van Loon and his colleagues applied this training to a pool of 49 nurse practitioners to understand the number of ultrasound-guided peripheral IV catheter insertions a participant needed to perform in the life-case setting to become competent.<sup>6<\/sup><\/p>\n<p>For this study, competency was defined as the ratio between the least amount of time in peripheral IV catheter insertion with the highest success rate on the first attempt.<sup>3,6<\/sup><\/p>\n<p>Dr van Loon found that the learning curve for ultrasound-guided cannulation was \u201cquite steep.\u201d<sup>3<\/sup> Nurses were considered competent after only 34 procedures, decreasing their insertion time from nearly 15 minutes to about 3 minutes.<sup>6<\/sup><\/p>\n<p>The training also resulted in a 93% success rate for first-attempt insertions.<sup>6<\/sup> Notably, this number raised to 94% for patients with difficult IV access.<sup>6<\/sup> Without the use of ultrasound, these patients had only a 6% success rate.<sup>6<\/sup><\/p>\n<p><strong>Learn more about proper peripheral IV catheter insertion: <\/strong><a href=\"https:\/\/eu.bd.com\/iv-news\/scientific-publications\/healthcare-worker-maintains-a-peripheral-vascular-access-device\/\"><strong>The European recommendations on the proper indication and use of peripheral venous access devices (ERPIUP) consensus<\/strong><\/a><\/p>\n<h4><strong>3.
