The choice of central venous catheter (CVC) is critical in oncology. As an oncology professional, you have multiple factors to consider. Different options involve different benefits and potential complications.<\/p>\n<h2>What are the advantages of PICC lines?<\/h2>\n<p>Oncological drugs and central venous access were developed after the end of World War II. The port catheter was invented around 1970.<\/p>\n<h3>PICC lines have been available since the 1970s, but their use has increased significantly since 2000 for several reasons<sup>1<\/sup>:<\/h3>\n<ul>\n<li>Ease of placement<\/li>\n<li>Ultrasound guidance ensures proper position<\/li>\n<li>Lower cost of insertion compared to other CVCs<\/li>\n<li>Lower risk of bleeding<\/li>\n<li>Ease of removal<\/li>\n<\/ul>\n<h2>What are the potential risks associated with CVCs?<\/h2>\n<h3>1. What is the risk of BSIs between different CVCs?<\/h3>\n<p>Chopra et al performed a systematic review comparing complication rates between PICCs and other CVCs<sup>2<\/sup>. According to the results, PICC lines placed in inpatients were twice as likely to cause <a href=\"https:\/\/eu.bd.com\/iv-news\/news-innovation\/the-real-cost-of-catheter-related-bloodstream-infections\/\">bloodstream infections (BSIs)<\/a> than those placed in outpatients<sup>2<\/sup>.<\/p>\n<h3>2. What is the risk of thrombosis between port catheters and PICC lines?<\/h3>\n<p><span data-contrast=\"none\">Thrombosis is more common in patients with cancer than those without cancer<sup>5<\/sup>. In systematic reviews conducted by Saber et al. and Yeow et al., it was found that thrombosis rates were higher in patients with PICCs than those with port catheters<sup>6,7<\/sup>.\u00a0\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">However, Chopra et al. suggested that the rates of thrombosis for PICCs may vary based on the techniques used to place them<sup>5. <\/sup><\/span><\/p>\n<p><a href=\"https:\/\/eu.bd.com\/iv-news\/news-innovation\/effect-of-technology-changes-%E2%80%A8insertion-of-picc-line-using-ultrasound-navigation-equipment-in-an-oncology-department\/\"><span data-contrast=\"none\">Ultrasound navigation equipment<\/span><\/a><span data-contrast=\"none\"> can be used to ensure placement in the appropriate vessel<sup>3<\/sup>.\u00a0<\/span><\/p>\n<h3>\u00a03. What are the rates of IV catheter-related complications for PICC lines?<\/h3>\n<p><span data-contrast=\"none\">Prof. Baudolino Mussa from the <a href=\"https:\/\/en.unito.it\/\">University of Turin<\/a> and colleagues carried out a prospective study on PICCs in outpatients with cancer<sup>3<\/sup>. Can PICCs be a safe method for administering chemotherapy?<\/h3>\n<p><span data-contrast=\"none\">In a seven-year study of 2,477 patients, Campagna et al. found that PICCs can be safe for administering oncological drugs in the medium- to long-term with low rates of infection and thrombosis<sup>8<\/sup>.\u00a0<\/span><\/p>\n<h2>Choose a CVC adapted to the specific needs of your patients<\/h2>\n<h3>When can port catheters be used?<\/h3>\n<p><span data-contrast=\"auto\">Mielke et al. recommend against using port catheters for less than 30 days<sup>4<\/sup>. They should not be placed for standard radiation protocols (less than\u00a060 days) either<sup>4<\/sup>.\u00a0<\/span><\/p>\n<h3>When can PICCs be used?<\/h3>\n<p><span data-contrast=\"none\">According to Mussa et al., PICCs may be used safely in oncology patients\u00a0if they are inserted and managed correctly<sup>3<\/sup>. Their duration can be extended from a few weeks to up to 6 months<sup>4<\/sup>. Unlike port catheters which require minor surgery for placement, PICCs offer the advantage of a lower risk of procedure-related trauma<sup>4<\/sup>.\u00a0<\/span><\/p>\n<h2><span data-contrast=\"none\">The bottom line: One CVC doesn\u2019t fit all<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559685&quot;:1440,&quot;335559739&quot;:160,&quot;335559740&quot;:240}\">\u00a0<\/span><\/h2>\n<p><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559685&quot;:1440,&quot;335559739&quot;:160,&quot;335559740&quot;:240}\">The same CVC cannot be used in all patients. It is important to choose the appropriate CVC based on the patient\u2019s needs<sup>9<\/sup>.
